FOSTER GUARDIAN CONTRACT

I,______________hereby agree to serve as a foster guardian for H.A.R.T. dog_______(name),  H.A.R.T. I.D.#_________ on this day of_____________, 2002.

By affixing my signature and initials to this document, I hereby agree to the following terms and conditions on what may be a temporary or long-term basis:

• During the period I serve as foster guardian for the dog named above, I understand that H.A.R.T. remains the sole legal owner of the dog and I therefore cannot lay claim on him/her unless I am willing to permanently adopt him/her under the provisions and restrictions already established by H.A.R.T.

• H.A.R.T. may terminate this foster guardian arrangement without notice or explanation.

• Upon termination, I will immediately relinquish the dog to H.A.R.T.

• While the dog is in my care, I understand H.A.R.T. will be responsible for the following approved costs associated with my foster guardianship:

1. Veterinarian medical fees

2. Medications or food supplements necessary for good health and well-being

3. Other to be agreed upon by H.A.R.T. and the foster guardian

• While the dog is in my care, I agree to provide:

         Wholesome food and water and exercise

    Care, attention and appropriate quality time

    A safe, indoor home with access to outdoors when and where   appropriate

    Appropriate structured routine in keeping with the dog's age and condition

    Administration of medications when appropriate to notify H.A.R.T. immediately of any physical, emotional or behavioral changes.                 

Under the terms of this Agreement you are not libel to us for any injuries to, illness or disappearance of the dog arising out of your foster guardianship except if such injuries, illness or disappearances are caused by or arise out of your gross negligence or intentional misconduct.  We are not libel for any bodily injury or property damage, losses or injuries whatsoever to you or other persons, or to you or another persons' animals and pets, caused by the actions, behavior or health of the dog, or arising out of the foster care, except if such damage, losses or injuries are caused by our gross negligence or intentional misconduct.
